Jill Adolphe and Sydney Graham

Jill and Sydney are Co-Founders and Partners of Care 2 Collaborate. We are Patient and Caregiver Engagement Specialists, with in-depth knowledge and expertise in Patient and Family Centred Care (PFCC), Engagement Education and the Patient Experience. Over 22 years ago, we began our journeys towards transforming the patient and family role within the health care system, inspired by our experiences as patients and family caregivers. Our respective health care journeys include caring for family members, and navigating the health care system through numerous surgeries, assessments, appointments, adverse events and transitions. We progressed from the roles of long time volunteers, Patient and Family Advisory Council Co-Chairs, to Family Lead Advisors. Given our unwavering passion to transform the Patient and Family Experience, we then joined forces as Consultants and Educators, founding Care 2 Collaborate and dedicating ourselves to empowering patients and family caregivers, and strengthening meaningful partnerships between providers, patients and family caregivers across the continuum of care.

Our extensive experience and insight gained from working with health care providers and organizations, academic institutions, government organizations, governance bodies, patients and family caregivers complement our national/international training and first-hand experience as patients and caregivers.

Our innovative, multi-faceted approach to collaborating with all stakeholders is not only elevating the patient and family caregiver voice, but it is continually forging new, mutually beneficial partnerships, thereby inspiring transformational change.

Our experience goes beyond the Ontario Health Care system, with Sydney having been an active member of the B.C. Children’s Hospital’s “Partners in Care” Advisory Committee. She created and chaired The B.C. Cleft Palate / Craniofacial Foundation. Sydney’s 12 years of work in the Radio and Television Industry enhances her public speaking, teaching and presentation skills, which she brings to her consulting work.

Jill holds an Honours BA in Economics from the University of Toronto, and a Graduate Degree (with Distinction) in Business from The London School of Economics (LSE), UK. She was employed in the Finance and Investment Industry for 17 years, which further developed her business acumen and strategic mindset.
